Its basically two lights in this setup, the main light is a 3/4 light that is its 45 deg to the right of the camera, the object of this light was to just give a small triangle shadow to the nose, this is achieved by raising the light until the shadow is similar to that in the render. I always use a strong main light. The other light is a fill light this is about a quarter to half the strength of the main light and adjusted to fill the dark area just enough to pick out shadow detail.. I may use a third fill light if I want to show more detail in the shadows. Generally its a variation of a 3 light portrait set up.
